How to Apply

First, follow the graduate steps to apply.

Then submit (mailed in one packet) to the DPT Department by December 15:

  1. Three confidential letters of recommendation
  2. Application (see the Doctor of Physical Therapy Program Web page)
  3. Statement of motivation, professional goals, experience and knowledge of the physical therapy profession
  4. Statement of purpose reflecting understanding of the challenges facing DPT practice and willingness to work with diverse clients in a variety of physical therapy settings

Incomplete applications will not be considered in the admissions selection process for the 2013–14 academic year.

Fall 2013 Deadline   

Step 1: CSUMentor Application—December 15*

Step 2 and 3: File completion—January 10*

*Applicants are encouraged to submit all required materials by December 15 but no later than January 10, to receive full consideration.

Spring Deadline
