Doctor of Physical Therapy, DPT

How to Apply

First, follow the graduate steps to apply.

Then submit (mailed in one packet) to the DPT program by January 15:

  1. Three confidential letters of recommendation
  2. Application (see the Doctor of Physical Therapy Program Web page)
  3. Statement of motivation, professional goals, experience and knowledge of the physical therapy profession
  4. Statement of purpose reflecting understanding of the challenges facing DPT practice and willingness to work with diverse clients in a variety of physical therapy settings
  5. Official transcripts (in sealed envelopes) from all U.S. postsecondary institutions attended

Incomplete applications will not be considered in the admissions selection process for the 2012–13 academic year.

Fall Deadline

Step 1: CSUMentor Application—February 1 (Applicants are encouraged to submit by January 15 to allow adequate time for collating application materials. Admission decisions will begin soon after the February 1 deadline.)

Step 2 and 3: File completion—March 1

All application materials including departmental requirements must be submitted by the file completion date.
