First, follow the graduate steps to apply.

Then, by December 1, submit the Graduate School of Public Health Application (via SOPHAS.org). Get complete application instructions for your program:

Fall 2013 Deadline   

Step 1: CSUMentor Application—December 15
Step 2 and 3: File completion—January 10

All application materials including departmental requirements must be submitted by the file completion date.

Spring Deadline

Domestic Applicants: Closed
International Applicants: Closed
